SOURCE OF AND REASON FOR REFERRAL

Rusty was referred to the agency by Suffolk County Criminal Court after receiving a DWI. Rusty was arrested for the current offense on December 1, 2016 with a blood alcohol level of .16. Rusty is court mandated to complete 90 days in outpatient substance abuse treatment.

CLIENT’S IDENTIFIED AREAS OF CONCERN

Rusty entered the program due to a court mandate. He states that he does not have any problems with substance abuse. Rusty disclosed that he was previously arrested at age 18 for possession of marijuana with intent to distribute, and at 22 for possession of cocaine. He states that he has not used drugs in over ten years, and has no history of participating in treatment. Rusty states that he drinks socially and rarely has more than four drinks. He states that on the night of his arrest, he was out with friends to celebrate a birthday and didn’t realize he had too much to drink.

ECONOMIC RESOURCES

Rusty chose not to disclose his personal finances. He works full time as a carpenter, although he believes he is at risk of losing his job due to a suspended license and lack of transportation to work.

He has active health insurance that he is hoping will cover the cost of his treatment. He states that his health insurance will be maintained through his union as long as he does not miss more than six months consecutively.

His wife works part time as a teacher’s aide. Her income was not reported.

RELEVANT MEDICAL HISTORY OF THE CLIENT

Rusty was hospitalized after a car accident was he was 27 and underwent surgery for a broken wrist which required pins. Rusty did not disclose any additional personal medical history, but did share that his mother in law, who is residing with the family, has breast cancer. She recently had surgery and is currently going for radiation treatments.

Rusty denies any history of psychiatric illness for himself or any family members.

Rusty has a history of using alcohol, marijuana and cocaine. Family history of substance use or addiction is unknown.

EDUCATIONAL HISTORY OF THE CLIENT

Rusty dropped out of high school in 10th grade. He received his GED at age 25.

WORK HISTORY OF THE CLIENT

Rusty is currently employed as a carpenter. He stated that he has been doing carpentry for most of his adult life, and joined a union at age 26. He states that he is laid off from time to time, and that the union finds him work. The longest period of unemployment that he recalls lasted three months.

SOCIAL/FAMILY HISTORY

Rusty lives with his wife, Jane, age 32, and their son, Ian, age 6. Jane’s mother, Patsy, also resides with the family. Rusty has a daughter, Ella, from a previous relationship. Ella is 9 years old and resides with her mother in Queens. Rusty reports that Ella’s mother does not let him see her very often, and states that there is no involvement from family court regarding visitation or child support, but states that he sends money when he can. Rusty states that he and Jane have been married for six years, and that their marriage is ‘ok.’ Jane works part time as a teacher’s aide at Ian’s school. Prior to that, Jane was reported to be a stay at home mom. Rusty states that Ian is “a good kid but kind of a mama’s boy.” Rusty states that he would like for Ian to start playing sports, but that it is expensive and Ian does not show interest. Rusty states that Patsy moved in about six months ago when she was diagnosed with cancer. He states that she does not contribute to the household due to her illness, and he is hoping she will get well soon so that she can move out.

Rusty shared little about his family of origin. He stated that his father died young, when Rusty was 6 years old. He states that he does not keep in contact with his mother or his two older brothers, and did not expand on when or why they stopped communicating.

Rusty states that he began experimenting with drugs and alcohol in high school, and describes his drug use as “typical teenage stuff.” Rusty denies that drug or alcohol use was related to his withdrawal from high school, and states that he always disliked school. He did not expand on factors that led up to his dropping out. Rusty states that he began selling marijuana at age 16 because it was “easy money.” He was arrested at age 18 in a parking lot with what he describes as a small amount of marijuana, but states that they charged him with intent to sell because what he had was “bagged up” and there was a pack of baggies in the car. Rusty states that he was given probation and got off at age 20 with no problem. Rusty was again arrested at 22 at a house party. He states that the party was raided by police due to underage drinking, and that he didn’t run because he was not underage. He states that unfortunately he pulled out his wallet in front of the police to show his ID, and a small bag of coke fell out of his pocket. Rusty states that the charges were later dropped when the officer who arrested him did not show up for court, and states he did not think it would show up on his record.

Regarding the current offense of DWI, Rusty states that he had a little too much to drink for a friend’s birthday, and was pulled over for swerving. Rusty states that he swerved because he dropped his cigarette, not because he was too drunk. Rusty states that his wife is not happy about the current circumstances, but believes that they will work through this. Rusty does not believe that outpatient treatment will benefit him, as he does not believe that he has a problem with drugs or alcohol, but states he will do what he has to in order to resolve the court case.
