Agency Plan – 125 points and constitutes a significant portion of the overall grade Each student will prepare an Agency Plan for developing a Human Services & Counseling related program which responds to identified community and/or population needs. The Agency Plan will be submitted as an APA paper (e.g., typed, double-spaced using Time New Roman 12 font, and numbered pages) with specific headings as noted below addressing the various components of the PROJECT. An Abstract is NOT necessary but be sure to include a reference page citing all of your sources. 2020-2021 Academic Year Part 1: Valued at 100 points DUE on or before 12/1/2020. The following is a list of required components for the Agency Plan. Additional information may be needed depending on the type of agency chosen. 1. Title of Program 2. Mission statement 3. Values statement 4. Purpose Statement 5. Needs Assessment a. Demographic data of the region to be served b. Rationale for the need of the agency 6. Vision and Goals 7. Implementation Plan 8. Community Interaction/Involvement 9. Organizational structure 10. Job descriptions for at least 3 distinctively different employees or anticipated employee named on the organizational structure (e.g., specialization requirements) 11. Marketing, Advertisement Plan and Materials (e.g., brochure, business cards, etc) 12. Policies and procedures (at least 5) 13. Advisory board or Board of Directors information 14. Volunteer information/requirements 15. Itemized Budget 16. Grant/funding information 17. Legislative information – what is your legislature doing for you in this profession? 18. Program monitoring and evaluation criteria and instrument 19. Citation of References
