Ms. T is a 44-year old who is being seen at the outpatient palliative care practice for pain management. Four years ago, she was diagnosed with rectal cancer and underwent surgical resection. When her disease recurred 2 years later, she decided to seek holistic and herbal treatments which were unsuccessful. Unfortunately, she experienced an intestinal obstruction and underwent surgery (transverse colectomy), chemotherapy, and radiation. In addition to her cancer pain, Ms. T has chronic pain related to rheumatoid arthritis and degenerative disc disease in her spine. She reports continuous pain in her lower abdomen and lower buttock, which is described as throbbing and stabbing. She rates her pain as a 7/10 at its worse and 2/10 at its best. The pain is now constant, exacerbated by standing, and has been getting worse over the past few weeks. Her pain management includes a combination of extended-release morphine 90 mg PO q12 hour, intermediate-release morphine 15 mg PO q4 hour PRN (6 tablets) with docusate and senna as a bowel regimen. Ms. T reports that this regimen had been working well, without any side effects, but now she experiences only 50% relief. Recently, she needed to go on disability from her cashier’s job because the pain “just gets so bad.” Based on her report, the medication regimen is changed to include extended-release morphine 150 mg 12h, immediate-release morphine 30 mg PO q4h PRN, and the addition and the addition of gabapentin 300 mg PO at night, with titration up to 600 mg PO TID.
